  1. Entry Age of Policyholder: 21 – 59 years old

  3. Payment: Monthly or annualized auto debit from BCA credit card for 8 years
  4. Coverage Period: hospitalization benefits including hospitalization in Intensive Care Unit for 8 years and Death Benefit for 10 years since the policy is in force
